Inside the front cover is a sheet that reads: 'Government Insurance Office NSW, Notice to Insured Employee, Workers Compensation Act 1926', signed by the General Manager J. McFarlane. It also lists the formula for calculating compensation. An index at the back lists the names of injured people alphabetically.
